The stress Tony is going through begins to take its toll, and the hit Junior has ordered doesn't help.
Yes, the visions of/with Isabella and his fight with Carmela about her were all figments of Tony's imagination (although the scenes of her just being irritated at Tony did happen, we can assume). You're right, it is confusing as written, but she wasn't real.

- I was also extremely confused by the Isabella plotline! Are we supposed to come away from this episode thinking that Tony not only hallucinated her on several occasions, but also hallucinated his fight with Carmela about her? It seems too elaborate and internally consistent for a hallucination.
- Because of that, I half-suspected that Isabella was some kind of spy, keeping an eye on the Soprano house while the Cusamanos were away (without them knowing) but I guess not? Help me out here!
