I think she suspected from the beginning - didn't Kate touch Sophie's face sometime after the first rescue just like the way Batwoman? And Kate didn't start using voice distortion tech in the Batwoman suit from the beginning. I mean, if anyone is going to recognize someone from the shape of their lips/chin, it should be an ex-lover.
